PUEEN Product Reviewer Program
INSTRUCTIONS

Fill out the form below in its entirety; all fields are required.  Answer truthfully and select your options carefully.  Product reviewers are hand selected and chosen by their listed preferences.  

**PLEASE NOTE**

1.  If chosen to be a product reviewer for PUEEN, by filling out this form, you are agreeing to write product reviews on Amazon.com and/or on pueen.com; you are also agreeing to post your review or art work on at least one of these social networks (Instagram, Facebook, YouTube).  

2.  Time frames for reviews are 15 days after product is received.  If reviews cannot be met within the time frames, it is the reviewers responsibility to contact their PUEEN representative.

3.  Per Amazon.com reviewer policy, each reviewer must have an existing Amazon.com account open that has at least (1) purchase made.

4.  Reviewers will be emailed directly for contact information if selected.  Due to the first-come, first-serve nature of this program, if an email response is not received within 3 business days, you will no longer be considered for future reviews and may be deleted from the selection list.

5.  To keep list refreshed, list may be purged occasionally at PUEEN's discretion.

PUEEN has the right to refuse product or make necessary changes to this program as seen fit.
