We request you to email the picture to youth@valleyranchmasjid.org. The YYP Administrators will use the picture to validate the child's identity when they are confirming his/her attendance at a certain activity. We may have up to 100 kids in the competition, and relying on just names will not be sufficient to accurately keep track. Some guidelines for the picture: Taken within the last 6 months to reflect your current appearance. Taken in front of a plain white or off-white background. Taken in full-face view directly facing the camera. Taken in clothing that you normally wear on a daily basis. Your full face must be visible, and the head covering must not cast any shadows on your face. Headphones, wireless hands-free devices, or similar items are not acceptable in your photo. The spirit of this competition is to encourage your child to participate in the activities/programs VRIC will have throughout Ramadan. While the prizes are nice to win, our real aim is to instill a love for the things that make Ramadan beautiful - fasting, praying, our community, etc. - in the child. By checking the "Yes" box below, you acknowledge that you have entered all of the information to the best of your knowledge and that you believe all of the information to be accurate. You also acknowledge that you will keep the goal of instilling a love Ramadan as the top priority throughout the competition and not the prize. *

By checking the "Yes" box below, you acknowledge that you and your child will abide by the rules established for this competition. VRIC and the YYP Administrators have the right to remove your child from the competition if any foul play, cheating, or misbehavior is observed. There is a zero tolerance policy on this matter. You will be notified via email if such action is taken with the details of the behavior that caused the disqualification. You will have a chance to respond, but you agree that the final decision will be in the hands of the VRIC YYP administrators.   *

Especially for parents of girls who are of age and may not be fasting due to their period, we are relying on you to accurately report some of the alternative activities they are doing. By checking the "Yes" box below, you understand that participation in the YYP Ramadan program is entirely voluntary and requires participants to abide by applicable rules and standard conduct that you have gone over and discussed with your child. You release VRIC, VRIC YYP, the activity administrators, youth mentors, all employees, volunteers, related parties and/or other organizations associated with the VRIC YYP from any and all claims or liability arising out of this participation. In case of emergency involving your child, you understand every effort will be made to contact you. In the event you cannot be reached, you hereby give your permission to the medical provider selected by the adult in charge to secure proper treatment, including hospitalization, anesthesia, surgery or injections of medication for our/my child. Medical providers are authorized to disclose to the adult in charge examination findings, test results and treatment provided for purposes of medical evaluation of the participant, follow-up and communication with the participant’s parents or guardian and/or determination of the participant’s ability to continue in the program and activities. You agree to indemnify VRIC and its representatives for any costs incurred in arranging medical care for our child under this provision. *
