Mountain Heights Academy Birthday Party
Please fill out this form to let us know if you are coming. We want to make sure and reserve an unlimited fun wristband for you! You will need to fill out a separate form for each Mountain Heights Academy student wanting to attend. Each student may bring one 7th-12th grade guest.

When: Tuesday, August 21, 2018 at 7:00-10:00 pm
What: Join us in celebrating our Birthday! Students will get an unlimited fun wristband for 3 hours and cake will be served.

Price for each additional guest and family member is $17.95 and can be purchased at Boondocks.
