Waiver Release Form(Mandatory)I understand and acknowledge that participating in the "Cinco de Mayo Rocket 5K Fun Run & Health Fair" is potentially hazardous, and that by entering and participating in the event, I am medically able and sufficiently trained to participate in the event. I assume full and complete responsibility for any illness, or injury or accident which may occur while I am traveling to or from the event, during the event, or while I am on the premises of the Event. I am also aware of and assume all risks associated with participating in the Event, including but not limited to falls, contact with other participants, and effects of weather. I, for myself, and anyone on whose behalf I am executing release, hereby waive, release, agree to indemnify and defend, and to forever discharge South West Independent School district and McAuliffe Middle School, and its sponsors, vendors, and volunteers, along with all their representatives, employees, directors, successors, and assigns (together, the "Releases Parties"), from any and all liabilities, claims, actions, costs, expenses, or damages, arising out of, or related to, my participation in the Event, even if such claim or liability arises out of the allegiance or carelessness on the part of the released party.I further grant to SWISD, vendors, and sponsors, a royalty-free, worldwide license and permission, with the right to grant sublincenses therein, to use my photographs, pictures, recordings, or any other record of this event for promotion or any other reasonable purposes related to the event. *
