The next time you go out to eat, order take out, or bring home left overs use this form to document all the single use plastic from the restaurant.

Single use plastic is just like it sounds; it is plastic that is designed to be used once and then disposed.

You are focusing on all single use plastic whether it is recyclable or not. These are items that were originally designed to be thrown away after being used once.

This form allows you to evaluate single use plastic whether you are eating in or taking out. Check all that apply. If you are uncertain how to answer these questions, feel free to ask the restaurant manager or server for help.

Your input is valued and will be kept confidential. You may evaluate as many restaurants as you wish, however, you will need to use a new form each time.

Together, we can encourage restaurants to reduce their use of single use plastic that ends up in our landfills and oceans.
