SGS does not grant access to people staying at Olofshöjd Veckobostäder (hostel) or outsiders.

First Name *

The name of the person with the SGS contract, in case you live in a multi-room apartment.

Last Name *

Same here. Last name of the person with the contract.

Block # *

Three digits of your address - for example if you live at Uppstigen 123, then it is 123.

Flat # *

In most cases it's two digits - your flat number (not the four digit postal number). If 01, just write "1".

Aptus # *

Five digits found on your blue plastic SGS electronic key.

Email Address *

To notify you when your gym membership has been processed.

Clear form

Never submit passwords through Google Forms.
