CMS Athletic Participation Waiver
MOFFAT COUNTY SCHOOL DISTRICT- CRAIG MIDDLE SCHOOL ATHETIC PARTICIPATION PACKET
Students participating in athletics at Craig Middle School must register and receive Athletic Department clearance prior to their first practice.
Athletic Participation packets are due at THE START OF THE SEASON.
Athletic participation is a privilege, not a right. Those who are interested in participation must
initiate the process as outlined below. The student athlete is the participant and is responsible for
gathering, completing and submitting this registration packet to the Athletic Department with all
of the following materials attached (an incomplete packet will not be accepted and will delay the
athlete from participation):
• Athletic Registration/Emergency Information form: A new registration form is
required for each sport, every season. Team managers must have a completed,
signed registration form to ride school-provided transportation.
• Athletic Sports Physical Form: This form must be completely filled out and attached
with this packet unless your sports physical form is already on file. A Physical is
effective for one calendar year from date of exam.
• Insurance Information: All CMS Athletes are required to maintain medical insurance
during the sports seasons. If the Student does not have medical insurance, his/her
guardian can purchase sports insurance through the school program
• Sports Fee: Your sports fee must accompany this packet unless you have met the
$140.00 family maximum amount. There is a $40.00 sports participation fee per
sport.
• Late Fees and Cleaning Fees: Athletes turning in their uniforms after the assigned
due date will be charged the following fees: $20.00 late fee, $20.00 if the uniform
needs to be cleaned. Football late fee (includes cleaning) $50.00. If a Uniform is not
returned to the school, a replacement fee will be assessed accordingly (including but
not limited to: 1 uniform, shipping, customizing, etc.)
• In addition to the above requirements transfer students and students attending
any other school besides Craig Middle School are required to provide: (1) Current
class schedule
• Athlete Tables: Student athletes that have one or more F’s may be required to
participate in a study hall/athlete table.
• Travel Release: This form is REQUIRED if the athlete intends to not ride the School
district vehicle home from athletic events.
• Eligibility Policy: Eligibility reports will be produced weekly from Monday to Monday. If
a student is deemed ineligible based on Monday’s report, that student will have until
Wednesday to raise their grade. If a student is unable to raise their grade during that
week, they will remain ineligible until the following Monday or until the following eligibility
report has been produced.
To ensure your participation on the first official day of practice you must submit this packet to the
Athletic Department two weeks prior to the season start date. Athletes registering late may miss
practices and/or competitions. Each day late = one day missed practice. Upon submitting this
packet, the Athletic Department will verify all eligibility requirements and send notification to the
coach that the athlete may begin practice and/or competition. Additional questions may be
directed to Athletic Department.
REFUND POLICY AND CONDITIONS FOR REFUNDS
All athletic fee refunds for cut/drop will be approved by the Athletic Director. The Athletic Director
will give a copy of the refund form to the school bookkeeper from which to process the refund. If
the athletic fee(s) were previously waived due to family maximum or free/reduced lunch eligibility,
there may be no refund or partial refund payment amount due. Students will be eligible for a full or
partial refund of athletic fees due to cut/drop only under the following conditions:

*Request for refund must be made to the Athletic Office. Refund will be pending
head coach’s verification and Athletic Director’s approval.
