本人同意 敝子弟參加由 貴機構主辦之「傑志守門員訓練班」，同時亦證明 其身體狀況良好，能應付本計劃的訓練及小型比賽。本人並明白 敝子弟在訓練及小型比賽期間或往返途中如發生任何意 外或損傷，主辦機構無需負責。參加者可按需要自行購買意外傷亡及保障財物保險。

 I agree that my son/daughter would like to participate in the “Kitchee Goalkeeper Academy” and also confirm that he/she is physically fit to meet the demand of the programmes’ training and competition. I understand the organizer will not be responsible for any accident or injury that may occur during the lessons and on the way to or from the training venue. Participants can purchase additional insurance to cover injury or death and damage to or loss of personal property according to their needs.
