Thank you for your interest in the inaugural New York State Farm to School Institute! Please read all the following information before completing your application.
We recommend having your application responses ready before completing this form. You will be asked to provide the following:
- General information about your school
- Names and contact information for a minimum of FIVE school team members
- Answers to the following Application Narrative Questions (250 word maximum for each):
1. Why is your school applying to the NYS Farm to School Institute? What does your team seek to gain from participating, and what can your school offer the other schools in the Institute? Please be as specific as possible in sharing your goals.
2. Describe your Farm to School program in relation to each of the 3 C's: Cafeteria, Classroom, Community&Farms (see FAQ for more information:
finys.org/institute). Please share any current or planned work in coordinating efforts within these areas.
3. What do you feel you need to take your program to the next level and how can the Institute help with these goals?
4. Please describe how you plan to measure your Farm to School program, both as a baseline at the beginning of the Institute and with projected increases as a result of your participation. This could include: projected percent increase of locally produced products to be procured per school; projected percent increase in the number of producers, distributors and/or food hubs supplying locally produced farm products to schools; or projected increase in student awareness of locally produced farm products.
