I am in a second SCHOOL sponsored extra-curricular activity and acknowledge that it is my responsibility, along with the band directors, coaches, sponsors, and parents to make sure that both activities have their needs met. If this does not apply to you, skip this question.
Select-sports teams, jobs, outside musical commitments, etc. are NOT considered excused absences.
I am aware of the attendance policy regarding unexcused absences.
1 unexcused absence allowed. More than 1 is grounds for and may result in removal from the cast.
I acknowledge that the “No Pass, No Play” date is the end of the 4th 6 weeks.  I acknowledge that if I am not at 70 or above in all classes (AP courses, Calculus courses and Dual Credit courses are exempt), my resulting academic ineligibly will result in removal from the production. There is no option to regain eligibility for this production.
