This is the annual inventory worksheet for Stages II - V AZELLA Placement Test Materials for Fiscal Year 2018. The AZELLA District Test Coordinator must annually submit this worksheet if additional Placement Test materials will be ordered for School Year 2017-2018.

If you are the AZELLA District Test Coordinator for more than one district/charter holder, you will need to submit this annual inventory worksheet for each district/charter holder entity which you will be placing an order for additional testing materials.

Inventory all AZELLA Placement Test materials for the DISTRICT. This includes all the materials at each school site.

Scorable materials are consumable and each student being tested will need one for their Stage of the Placement Test.

Nonscorable materials are reusable and one per student is NOT required.

If your district does not enroll students for a specific AZELLA Stage, please respond with 0.

This is not an actual order for materials. All AZELLA Placement Test materials must be ordered through PearsonAccess NEXT by the AZELLA District Test Coordinator.
