2018/2019 New Family Registration
To be considered registered the following steps must be taken as part of our admissions process:
1 - Meet with the principal for a campus tour
2 - Complete this registration form
3 - Sign up for a tuition account with SMART Tuition (enroll.smarttuition.com, School Code 12091, Make sure to select the 18/19 school year)
4 - Sign up to receive email communications from the school (http://www.stpatrickparnellschool.org/news/). IF YOU DO NOT ENTER EMAILS, YOU WILL NOT RECEIVE SCHOOL COMMUNICATIONS.

I agree to pay the tuition assigned to my family, as accounted for and maintained by SMART Tuition from July 1st to June 30th. I understand that the SMART fee of $35 (first month) and my registration fee (over 12 months) will be applied in the SMART system. *

I agree to assist with a minimum of two (one for Preschool only families) volunteer/fundraising activities each school year. I understand that the school office will send out a sign-up genius for me to sign up for specific activities. If I do not sign up, I understand that I will be assigned activities by the school office personnel. *
I agree to the following lunch/recess responsibility: In order to assure the safety and well being of students during lunch/recess, we schedule two to three K-8th grade parents (not Preschool) per week to assist with supervision. An initial schedule will be made available in August and will be followed by a complete schedule later in September. Parents who are unable to assist when they are scheduled are asked to find a substitute. Please inform the office of the sub's name. We will publish a list of parents who are willing to substitute. The fee for substituting is $8/day. The check should be made out to the sub, not Saint Patrick School. *
I agree to the following: To insure the protection of all God's children, the Diocese of Grand Rapids requires us to conduct a background check on every person who will be involved with our students. Please provide us with the information of each parent who be volunteering or spending time with our students. If you haven't already filled out the background check form, we will give one to you at the beginning of the school year. We must have this information for all who chaperon, visit classrooms, do lunch duty or otherwise interact with the children *
I agree that if my child participates in Saint Patrick School athletics, I will have have additional volunteer requirements to help with staffing home games, running concessions and helping with fundraisers. I understand that these efforts help to pay for all the athletic programs at Saint Patrick School. *
As part of Saint Patrick School's pest management program, pesticides are occasionally applied. You have the right to be informed of any pesticide application made to the school grounds or buildings. In certain emergencies, pesticides may be applied without prior notice, but you will be provided notice following any such application. Do you choose to receive prior notification if pesticides are applied? *
