Viva Brasil Association in St. Louis (“Viva Brasil”) sponsors and organizes the Activity referenced above. The Participant’s participation in such activity is subject to the terms set forth in this Parent/Guardian Agreement (“Agreement”):

Code of Conduct
The undersigned parent or guardian gives the Participant, a minor (child under the age of 18) permission to participate in the Activity. The Participant is required at all times to exhibit appropriate behavior, and may be discharged for any reason, including disruptive, dangerous behavior or failing to following instructions of the Activity’s leader.  Parents/guardians will be responsible for notifying staff of a child's absence, late arrival, early or late pick-up and any special circumstance.

Consent and Release
By participating in Viva Brasil activities and programs, the undersigned parent/guardian agrees to assume financial responsibilities of the Activities. The parent/guardian further agrees to release and hold harmless Viva Brasil and its staff members from any and all claims, suits, losses or related injury or death, accident or otherwise, during or arising in any way from the activities or programs of Viva Brasil, including damage causes by the Participant to facilities or materials utilized for the Activities.
Viva Brasil may use photographs of participants for publicity purposes.

Payment Provisions
• Payment is non-refundable and due at time of registration.
• Any other situation will be discussed on a one-on-one basis.

Cancellation
Viva Brasil may terminate the right of the Participant to participate in the Activity for any cause. Viva Brasil may also cancel or adjust scheduling or location of the Activity in its sole discretion.

Emergencies
In the event of an emergency, Viva Brasil or the Activity Leader, when appropriate, will first attempt to contact the parent/guardian based on the information below. In the event the parent/guardian is not available, the parent/guardian authorizes Viva Brasil to contact the individuals specified as emergency contacts below:
