Please use this form to log Volunteer Hours for the 2018-19 school year.  Please follow the following rules when filling out this form:

1. Use one unique First and Last name to identify your family unit.  Use the same First and Last name no matter who actually volunteered so that all of your family's hours can be credited to the same family.  This includes any and all parents, students, or family members that volunteer.  Please do not use only your first name and please do not use just your last name unless you are 100% sure you are the only family with that last name in the school.

2. Please enter hours by rounding to the nearest .25.  Please enter ONLY numbers.  Do NOT use the words HOUR(s) or MINUTES - Numbers only.  Example: You volunteered 1 hour and 15 minutes - enter it as 1.25.  This makes it easier to add up hours when you need to know the total.  :)
