I have cf , but also a daughter. I like her very much, and to help her understand my disease, I'm writing a fairy tale book. If you have cf, and would like to become a character,  please fill in the form.  

If you don't have cf, but know/knew someone who has/had, you can also fill in the form with his/her answers.
I would like to remember with this book to those of us who has already got their wings.
