Use this form to nominate Outstanding PDS Mentor Teachers, Co-Teaching Pairs, Supervisors, Liaisons, or Site Coordinators. You may complete a nomination form in more than one category and for more than one person in each category. However, please access a new form for each nomination.  
Nominees that have been honored within the past five years are not eligible for the award this year.  For a list of past recipients, visit http://www.salisbury.edu/pds/about/rpds-awards.html.  All nominations are due by midnight April 15, 2016.  You will be notified by email if your nominee is chosen for an award.

Criteria to consider for Outstanding Intern Supervisor
•Collaborative

•Professional- reliable, collegial, and fair
•Visibility- consistent visitations
•Feedback- timely and useful and completes all evaluations
•Supportive of intern and mentor

Criteria to consider for Outstanding Liaison
•Communication
•Professional- reliable, collegial, collaborative, and completes documentation
•Visibility- consistent presence at the site/s
•Supportive of school and university initiatives

Criteria to consider for Outstanding Site Coordinator
•Communication- with school and university personnel
•Professional- reliable, collegial, collaborative, and completes documentation

•Visibility- regularly attends school and university PDS activities and meetings
