National College Health Assessment (NCHA)
 WE NEED YOUR HELP COLLECTING IMPORTANT HEALTH DATA RELATED TO STUDENT HEALTH AND WELLNESS.

Every 3 years, Fullerton College Health Services surveys 1,000 FC students in the spring semester using the National College Health Risk Assessment (NCHA) survey.  NCHA measures risk factors which impact academic performance and retention such as: safety and violence, mental health, alcohol, tobacco and drugs, sexual behaviors, nutrition, physical activity, and has allowed us access to health information and health habits.

Request:
Health Services has been asked to re-survey 1,000 Fullerton College students in spring 2019 using the National College Health Risk Assessment survey. The survey will be administered by health center staff throughout the month of MARCH 2019.  It will take students about 45 minutes to complete the entire survey. We are asking for you to reserve only one day during the month of March to allow us to come into your classroom and conduct these surveys.

To reserve your date and time, please complete the form below with you course #, day in March your class is available and time (survey takes around 45 minutes). Please list each classes individually.

We will follow up with you and confirm our attendance to facilitate the survey by email as soon as possible. We appreciate your support!
