BOYS BASKETBALL

If your son plans to play middle school basketball, there will be a mandatory parent meeting after tryouts on Tuesday, October 25 at 7:00 PM in the cafeteria. It is imperative that parents attend this meeting..

I agree that any athletic uniform issued to this child will be properly cared for and returned by Monday, Feb. 13, 2017 in clean and in good condition.  I understand that a $100.00 fee will be charged for any and every school-issued uniform not returned by the deadline.   *

Parent/Guardian Consent & Medical Physical Forms
A Parent/Guardian Consent Form must be completed for each student athlete in middle school.  These forms are available on ParentWeb>Resource Documents or in the school office.

Middle school (6th-8th) student athletes must undergo a medical physical administered by a licensed physician to play fall sports at St. Vincent de Paul Catholic School.  Please have the physician complete the school's Medical History & Physical Form.  The physical must have been administered after May 31, 2016.

Final boys basketball rosters will be posted Tuesday, Nov. 1, 2016.  This online form and all necessary medical forms are due by Monday, Oct. 31, 2016.  Early submission is highly encouraged and appreciated.
