This is an optional form for parents to fill out. We have a very purposeful, carefully thought out process for student placement. We apply a team approach including input from the current classroom teacher. If you choose, fill this form out and submit by June 9, 2017. Forms that include teacher names will not be processed. Please complete this form online and do not turn in paper copies. Thank you.

Placing students in classrooms is a team effort involving classroom teachers, parents, specialists, and the school principal. Teacher assignments will be shared in August.
