Change your Certification Officiating Identity
Officiating Identities are needed for officials who are part of the certification system. They have four components: An officiating name, a league affiliation (which does not have to be with a WFTDA or MRDA league), optional pronoun, and an email address.

This identity is both how Certification will identify an official, how Certification will list a Certified Official online, and how other individuals will use Certification's documents to fill out Evaluations and Overviews of Officiating Skill for that official.

Publicly this will appear like so: "Vanna Whiteboard (she/her or they/them; Wheels of Fortune)" or if a pronoun is not on record, "Vanna Whiteboard (Wheels of Fortune)"

This form is used to request a change of name, pronoun, league, or email address.

This process takes approximately two weeks. During those two weeks, BOTH identities will appear on all of Certification's documents and forms, but as part of the conversion process any OOSes or Evals that use the old identity will be converted, so there is little risk during the conversion period. However, people filling out OOSes or Evals may become confused, so be sure to tell them to use your new identity, if you are asking for an OOS or Eval.

If you do not yet have an identity, use this form to request a new one: https://goo.gl/forms/pVCI2o9uTuPEfo6J2

To begin, enter your email address. Use your NEW, CURRENT, FUTURE email address if you are changing your email as part of this session, or re-enter your current longstanding email address if you want to keep it. Changes to email will have both addresses automatically emailed to verify that this is acceptable.
