2019 HSC Sponsorship Form

Please complete this form to become a sponsor of a Huntsville Soccer Team (or teams!) in the 2019 outdoor season.

Cost for team sponsorship is as follows:

     * $310 per HOUSE LEAGUE (LTPD) Youth or Adult team
     * $500 per REP (travelling team) who compete in the Huronia District Soccer League

Your sponsorship will be confirmed upon receipt of payment (payment instructions are at the end of this form).
