Please complete this form if you would like GSD Nutrition Services to provide a bag lunch for your child on the October 26,  2017 early release day.  This is available to ANY student, whether going to child care or heading home. PLEASE NOTE: lunch is not provided at the Y-Care after school child care, so you will need to order a lunch or send one with your child that day.

You must complete a separate form for EACH of your children.  Forms are to be completed by Friday, October 20 for the October 26 early release day.

The price of lunch will be deducted from your family lunch account. You do not need to send any money; just make sure your lunch account has a positive balance. If your family qualifies for free or reduced price lunches, you would receive this lunch for free or at the reduced price.  

Your child may select a wrap/sandwich and milk from the choices listed below.  

In addition to a sandwich and milk (please make your choices below), each lunch will include:
• a juice box, carrots or other raw veggies
• applesauce cup or raisins
• bag of snack chips

Lunches will be delivered to your child’s classroom prior to the time of their departure; lunches and milk will be kept cold.
