Hello!
This form serves as the official Creative Request Form for CFC-Youth Pacific. The form itself is to help us push and collaborate with the rest of Pacific Region in being able to evangelize through creative performances at numerous CFC-Y events. Whether it be a camp , phase event, Chapter Assemblies, conference. The YCOM production heads are here to help you organize it in order to make the event run more smoothly but also to keep your "creative" team accountable.

CREATIVE SPECIFICATIONS
When filling out the form please remember to include any specifications you may need for any of the creatives. These include a certain "image" you may want in the creative. Things like, costume , or even a rough storyline for a creative that you can't put together by yourself. We are here to help and guide you to a finished product and portray what you envision. So please include any links to songs you may want to use or any links to were able to give you inspiration. The more things we have the better we are able to see what you are thinking.
* if you are using an old creative the vision/explanation section does not need to be filled out. Please just include a video link to the creative.
In order for us to be able to communicate and organize everything as efficiently as possible please fill out the form with detail and any personal specifications to your creative(s).
