Welcome back! This form is made for existing instructors to propose their courses for upcoming quarters.

Please submit a separate form for each course proposal. Only electronic proposals are accepted.  Please answer all questions, then provide attachments as necessary.

Please do not contact the TELOS office regarding the status of your proposal. You will be notified via e-mail if your proposal is accepted. If your proposal is accepted for further review, you will be contacted by the department to discuss your proposal further.  Courses will not be considered without completion of this form.
