Thanksgiving Meal

Hello Parents & Guardians!

The information from this form will be used to pre-print visitor badges for our Thanksgiving Meal thus making your entry into Forest Creek that day more efficient. Every adult visitor will need their own badge.

The Thanksgiving Meal will be on November 21st. Please fill out the information in this form by NOON on November 20th. This will allows us time to process the badges and prepare them for the next day.

The lunch price is $3.85, payable upon arrival. Only cash and checks are accepted. Debit and credit cards are not accepted. Your child's class will eat lunch during their regularly scheduled lunchtime. A schedule of the lunches can be found on our webpage.

Please list all the name(s) for every adult visitor as it/they appears on driver's license. If you have had your ID scanned and printed before, just show your ID to pick up your badge. If not, come to the office to sign in.

You may list more than one student.

Teacher(s) Name *

You may list more than one teacher if you have multiple children at FCE and will be staying for lunch with both classes.
