APF Participant Request Form 2017
All participants including APF committee members, delegates, alternate delegates and attendees in any official capacity should complete this form whether requesting funding or not.  

If your community does not yet know who your Delegate will be in March 2017, and your community is likely to attend, please submit this form now with your current/previous delegate information so we can continue with our planning.  Once any changes are confirmed, please then email apfadmin@nzna.org.
