Nominations are to be submitted by completing this Bevan Medal application form no later than Tuesday 30 April 2019.

Forms will be submitted automatically to the club administrator for registration upon completion of this form.

There are three categories a member may be nominated for:

a) Senior Life Saver of the Year
b) Cadet Lifesaver (Under 17 – Under 19 age groups)
c) Junior Lifesaver (Under 14/15 Age Groups)

3. Nominees must hold the appropriate club membership category & awards e.g. Active/bronze holder (this may be confirmed with the club administrator)

4. Nominations can be made my any active member or office bearer of the club.

5. Nominations will be reviewed by the SHSLSC Board of Management to provide additional supporting information.

6. Nominations will then be forwarded to the Bevan Medal Selection Committee for consideration.

7. Winners will be announced at the Secret Harbour SLSC Awards Night on the 27 May 2017 by the Bevan Medal Selection Committee.
