First Week of School Transportation Form for OCRS 2018-2019
All of us at OCRS are so excited to see your students on the first day of school!

In order to avoid first day/first week dismissal confusion, we created an electronic  transportation form. Please carefully answer each question and complete each section. We will dismiss your child on the FIRST DAY according to the information you provide on this document.

Please complete A SEPARATE FORM FOR EACH OF YOUR CHILDREN and submit each separately.  Thank you for your help ensuring that each student is dismissed properly and safely!

On the first day of school you child will come home with an envelope that contains many forms including a BLUE TRANSPORTATION FORM.  Please fill it out and return it to school on the second day of school.  We will follow the dismissal plan you indicate in this document for the first FOUR DAYS of school and then we will follow the plan indicated on the blue form for the remainder of the school year.
