By completing this form, I (parent/guardian) confirm that we (myself and student) have read, understand and agree to abide by the Commencement Guidelines and the expectations for commencement. Student must successfully complete academic and attendance requirements from Glenbard West High School (as specified in the Castle Keys Handbook) during the first or second semester of the 2017-2018 school year and cleared all financial obligations by 2:45 p.m. on Wednesday, May 17, 2018. We understand that students may be removed from the commencement ceremony if they engage in inappropriate behavior during the commencement ceremony. Response are due by April 27, 2018
