Official HS Transcript Request Form
** HS students and parents can download unofficial transcript directly from the Parent/Student Portal. To get a copy, log into the Student/Parent Portal, click Reports >> Transcripts >> Print Unofficial Transcript.

If you require an official signed transcript, please complete this request form. Please submit one form per student in grades 9-12 only. If you should have any questions, please contact our High School Department at transcripts@sageoak.education or (888) 435-4445. Thank you!

Note: We do not provide transcripts for kindergarten through eighth grade students - even if they have taken a course in middle school for high school credit. Parents may request copies of the report cards from their K-8 teacher.
