SNSWUA - Candidate Application Form
This form is to be used for Softball Australia (SAL) Level 2, 3 and 4 National Accreditation candidate applications within NSW.

This form is to be submitted to the Softball NSW Umpires Association at least 6 months prior to examination.

All candidates must have a current theory pass, be a current financial member of SNSWUA and have attended a SNSWUA Umpire Development Clinic in the last 24 months.

Applications should be received before the closing date.
