Hello! This is the application form for PANTHEON, an online reality game styled after the CBS reality show SURVIVOR. Please fill out the following questions to the best of your ability and be as detailed as possible! Please do not send an incomplete form.

This application is split into three pages - Introduction, which is where you will provide basic information, Gameplay, which is where you will make your case on why you should be cast, and Competition, which will help explain the game and ensure you understand how everything will work.
