Summer Shindig 2019 Vendor Application
Vendor Space or Truck Fee is $150 or $100 if completed by April 1, 2019.
REQUIREMENTS:
• Completed vendor application and contract
- Submission of application does not guarantee acceptance to the Festival.
- You'll receive an email confirmation of your acceptance by April 9, 2019.
• Funds payable to Marlon Eilts, Financial Services, Inc. for your selected vendor space fee.
- Credit Card, PayPal, Check or money orders accepted. Returned checks will incur an additional $25 fee.
• Copy of Food Service Permit
- The person listed on the certificate provided must be present at the booth at all times.
• Copy of Certificate of Insurance
- List Marlon Eilts Financial Services, Inc., 315 N. Ladd St., Pontiac, IL 61764 as additional insured.
- All food vendors are required to have general liability insurance with at least $1 million limit.
