Request to Serve Wine and Beer on SOU Grounds
Use this form to request approval to serve beer and wine at an SOU campus event or off campus event. This form must be approved before an event utilizes the SOU Liquor License. If an event has a different licenses, this form must still be utilized, but the licenses/special license must be sent to the Campus Designee and the form must be completed for approval to be granted.

SOU's OLCC Campus Regulations
All Campus:
1.  All events being held at SOU that intend to serve beer and wine must notify the OLCC Campus Server Education Designee (Campus Designee) by utilizing the “Request to Serve” document located on the last page of this document.
2.  Approval is based on location, attendees and type of event. Should an event have Students or numerous minors in attendance, the service of beer and wine will be reviewed by the Executive Council at least 30 days prior to event date.
3.  Only beer and wine products are allowed to be served on campus for any event or function. Distilled spirits are prohibited by the type of license the University holds and may not be served on campus at any time.
4.  All beer and wine that is served on Campus must be served by an OLCC Service Permit holder(s) and the permit must be current and presentable prior to the function and during the event. Please note that the Campus Designee may require proof of licensure prior to or during the event by all who are serving alcoholic beverages. New Alcohol Service Permit Application Portal Coming May 1, 2018. OLCC is moving to a web-based application portal for Servers, Bartenders, anyone who serves alcohol by the glass or fills growlers, and the people who manage them.   As of May 1, 2018, all Alcohol Service Permit applications will be submitted through the portal, hosted on OLCC’s website, www.oregon.gov/olcc (new portal link will be available May 1st). Applicants are still required to take an in-person or online Alcohol Server Education course and must upload a proof of class document before taking the test through the portal.  There is no fee to take the test. Once the applicant submits an application and pays the $28.65 application fees, they will automatically get their temporary permit.  After staff have reviewed their application, if it is approved, the applicant will get an email notification to print the five-year permit.



Non-SOU Group Policy
In addition to "All Campus" regulations, all non-SOU groups must have the following.  

Outside Groups: to be completed by event organizers:

1. Insurance Policy with liquor liability of $2,000,000.00, send to Campus Designee (Sherrill@sou.edu). Insurance  Note the $2 million per occurrence and $4 million aggregate is required by the State of Oregon.  SOU requires all independent contractors provide a Certificate of Insurance, with SOU named as additionally insured, showing coverage to these limits.  Insurance may be available through a TULIP program. 2. Event name change, send update to Campus Designee (Sherrill@sou.edu)
3. Copy of OLCC license of bartender, send to Campus Designee (Sherrill@sou.edu)

All must be completed one week prior to event for event approval.

Due Date for all documents: Seven days prior to event
Campus Designee Review: Six days prior to event
