If you have more than one child and they have the same feelings/experience of 'Home from Home', then you only need to complete this survey once.  It would seem silly to expect you to complete it twice.  However, if you have more than one child and they have different feelings/experiences of 'Home from Home', e.g. one enjoys it but the other really doesn't, then please complete the survey twice.

Group responses may be discussed with members of 'Home from Home' staff and Governors, however all responses will be anonymised during these discussions.  

At the end of the survey, you will be asked whether you would like to add your name and e-mail address - this is essential if you would like a reply to any of the points which you have raised.  If not, then all your responses will be anonymous.

Once you have submitted the form, please collect a raffle ticket from Helen or Debbie (they will know you have responded but  they will not know your reponses).  The draw will take place on Friday 5th October with the winner receiving a bottle of wine!
