Decathlon Malaysia Sdn Bhd has the rights to use all photographs and videos taken during the DECARIA CUTI SEKOLAH event for the company's printed collaterals, website and/or social media platforms.

The member recognizes the difficulties of the Event and attests that he/she is sufficiently fit and able to take part in the event and has not been advised otherwise by a qualified medical professional.

Please be informed that Decathlon Malaysia Sdn Bhd will collect and use your personal data for purposes including but not limited to the performance of our obligations in relation to the purposes for which you have provided the information as well as for marketing purposes.

For and in consideration of Decathlon Malaysia Sdn Bhd permitting the member to enroll and participate in DECARIA CUTI SEKOLAH, the member, by signing this document, hereby voluntarily indemnifies release from liability, and holds harmless Decathlon Malaysia Sdn Bhd and the management for any accident, injury, illness, death, loss, damage to person or property, or other consequences suffered by the member or any other person arising or resulting directly or indirectly from the member's participation of the event organized by us. In the event, that the member is injured, the member agrees to assume any financial obligation, either through the member's personal health insurance, or through some other means, for any medical expenses, injury, or damages suffered by the member in connection with the use of any facilities or services in connection with being a member.

It is the intention of the member by signing this letter to expressly assume all risk of personal injury, death, or property damage upon him/herself, to the exclusion of Decathlon Malaysia Sdn Bhd and to exempt and relieve Decathlon Malaysia Sdn Bhd from any liability for personal injury, property damage or wrongful death.

The member further agrees that the member, his/her spouse, assignees, heirs, guardians, and legal representative will not make any claim against, sue or attach Decathlon Malaysia for any loss or damage resulting from the member's participation in the activity.

The member is aware of the potential dangers incidental to engaging in fitness activities. The member agrees that this is a release of liability, a waiver of the member's legal right to collect damages in the event of injury, death or property damage and a contract between the member and Decathlon Malaysia and the member signs it of his/her own free will.

I represent that I am eighteen (18) years of age or older and fully understand and accept the above terms and conditions, warning of risk, assumption of risk, waiver and release of claims for myself and/or the minor children in my care (where applicable).
