The following registration form will provide you with a secure, fast and easy way to sign up for all HIV/AIDS education programs (including certification courses and annual updates) in the central panhandle region. The schedule of dates and times for each program will be available to you during this registration, so you may have the option to choose one based on your own schedule or distance of travel. You may register for any class at any time prior to seventy-two (72) hours from the start of the course.

All programs are offered for free and are provided by a certified health educator with the Florida Department of Health.

You must complete each page of this form and hit "submit" at the end to ensure proper registration. Once you have submitted, you will receive a copy of the completed form immediately, as well as confirmation from the trainer (normally within seventy-two (72) hours) with a calendar appointment and any further instruction you'll need prior to class.
