2018 Arts Alive Award Nomination
Thank you for taking the time to submit a nomination for the first Arts Alive Award presented by Summit Artspace.

The award recognizes an individual or individuals who give selflessly to the cause of advancing arts and culture in the community.  An artist, arts manager, volunteer or patron, this individual is a true lover of the arts who gives willingly and does so without seeking recognition. The individuals must live in Summit, Cuyahoga, Portage, Stark or Medina counties in Ohio.

DEADLINE TO SUBMIT YOUR FORM IS MAY 11, 2018.

You may complete the online form below or print, complete and submit to: Executive Director, Summit Artstpace, 140 E. Market St., Akron, Ohio, 44308. Please be as detailed as possible when describing why the artist qualifies for the award. Include any verifiable online sites or other sources.
