Public Lab Code of Conduct reporting form
Public Lab values the space that we collectively maintain during our in-person events. We address concerns with thought and care and take reports seriously.

If you experience or witness something that may be a violation of our Public Lab Code of Conduct (available at https://publiclab.org/conduct or paper copy on the info table at the event), you may contact a member of the Conduct Committee for support or guidance either in person, via email at conduct@publiclab.org, or via this anonymous form. The current list of members of the Conduct Committee is available at https://publiclab.org/conduct

Submitting a report via the form sends an alert email directly to all members of the Conduct Committee.
During Barnraisings, email will be checked at minimum twice daily by at least one member of the Conduct Committee.
During the rest of the year, the email will be checked once weekly.

Describe the issue or cause for concern, in as much detail as possible. *
Who was involved in this? Who should we begin talking to? *
We ask about urgency so we know if a report requires immediate response; for example, we might take immediate action even at midnight on an urgent report.

(If there is an emergency and there's a threat of imminent harm, contact police or law enforcement immediately.)

VERY HIGH: our response will begin immediately upon receipt at any hour of the day
HIGH:            our response will begin immediately between the hours of 9am-9pm
MEDIUM:      our response will begin within 24 hours
LOW:             no immediate response required; follow-up within a few days

What is the level of urgency? *
If you want one of us to contact you, tell us the best way.
