Education Workshop #3 "Discovering Yourself: What Does Being a Physiotherapist Mean to You?"

• Registration is on a first-come-first-serve basis subject to availability of seats.  
• Limited to 30 seats only!    
• Registration fee:       $15 for SPA Student members
                             $30 for SPA Members
                                             $60 for Non-Members
• Target Audience:      Physiotherapy students and new graduates of 1 to 2 years of working experience.

Registration closes on 21 Oct 2017.


CANCELLATION POLICY AND DISCLAIMER CLAUSE
SPA must be informed of any cancellation or replacement in writing via email.  A full refund will be made if such notice is received by SPA before 21 Oct 2017.  There will be no refund of registration fee for cancellation after 21 Oct 2017.

SPA reserves the right to cancel or postpone the workshop, subjected to take up rate of participation. SPA and facilitators will not be held liable for any injuries, disabilities, death or loss of properties during the course of session.


PHOTOGRAPHY AND VIDEO/AUDIO RECORDINGS
Photographs and/or video and audio recordings may be made during the session. By attending the Event, you consent to:

(a) being interviewed and/or photographed;
(b) audio recordings and/or video recordings being made of you;
(c) the release, publication, exhibition, or reproduction of the above mentioned interviews, photographs, audio recordings and/or video recordings for use in any news articles or programmes, web-casts, promotional purposes, telecasts, advertising, inclusion on web sites, or for any other purpose(s) related to Singapore Physiotherapy Association or its representatives deem fit.
