    examination sitting.

    a. to help us understand the possible reason(s) for student's failure or pending result, and
    b. to know the recommended course of action and provide the necessary assistance for
        the student to complete their program.
3. This form is strictly confidential and is to be completed by the student's lecturer, program
    coordinator or academic counsellor.
4. The student's Information Declaration Form (IDF) is attached for your reference.
