HOPKINS COUNTY REGIONAL CHAMBER OF COMMERCE 2018 AWARDS NOMINATION FORM & GUIDELINES

NOMINATIONS DUE BY 4 PM ON JANUARY 7, 2019

The Hopkins County Regional Chamber of Commerce will honor individuals, businesses, and organizations during the 6th Annual Evening of Stars on Saturday, February 2, 2019 for contributions to the vitality and quality of life of the Hopkins County area in 2018.

The criteria to be used by the selection committee for the Horizon Award, Small Business of the Year, Business of the Year and Non-Profit of the Year are as follows:  1. Must be an active Chamber member in good standing, and do business in the Hopkins County area. 2. All nominations must be in writing and include information to support the nomination.  The nominating person or organization must include the nominator's name, address, and phone number.  A list of active Chamber members can be found at https://www.hopkinscochamber.com/members-alphabetically.html.  

The criteria to be used by the selection committee for the Corum Community Service Award are as follows:  1.  Must be an individual, business, or organizations who has performed outstanding service or made exceptional voluntary contributions to the community and exemplifies strong community values and ethics.  2. All nominations must be in writing and include information to support the nomination.  The nominating person or organization must include the nominator's name, address, and phone number.

Horizon Award - Must be a business in operation less than three years.
Small Business of the Year - Must be a business with fewer than 50 employees.
Business of the Year - Must be a business with 50 or more employees.
Non-Profit of the Year - Must be an active 501-C3 or C4 organization that has made a notable impact on the quality of life for those who live and work in the area.
Corum Community Service Award - Must be an individual, business, or organization who has performed outstanding service or made exceptional voluntary contributions to the community and exemplifies strong community values and ethics.
