MUNI Meetup Registration Form
Thank you for your interest to join our upcoming MUNI Meetup on Sustainable Fashion on June 16, 2018, 2-6PM at Clock In BGC High Street, 3/F C2 Building! Please fill in the details below to pre-register. :)

Deadline of registration and payment is on June 13, 2018 (Wednesday).
Make sure to book early and complete your registration by filling out this form + sending payment confirmation via events.muni@gmail.com.

WORKING PROGRAM:
- 2:00PM-2:30PM – Doors open for registration / pop-up shopping
- 2:30PM-5:00PM – Program proper (Introduction by MUNI; Panel & Q&A; Snack break; Group discussion; Summary + Action steps)
- 5:30PM onwards – Networking / pop-up shopping

TICKET PRICES:
P500 – Meetup Admission only
P900 – Meetup Admission + Thrive x Muni Tote*

Inclusions:
- Snacks and drinks (please bring your own tumblers)
* The THRIVE x MUNI upcycled cotton tote is made with textile handwoven in Bicol with respun shirt collars and other fabric scraps.

Walk-ins will NOT be permitted on at the venue on the event date.

Please note that registration is complete only upon completion of this form + submission of payment confirmation via events.muni@gmail.com.

Tickets may be paid via BPI Bank Deposit to MUNI Cultural Creatives Inc. #2673-0078-16 or via PayPal payment to hello@muni.com.ph (add P50/ticket for Paypal). Upon bank deposit / funds transfer, kindly send us proof of payment via events.muni@gmail.com by June 13 to complete your registration.
