This Leave of Absence form is for the Nine Tenths Mentoring Programme.

In cases where students cannot meet their voluntary commitments, they are required to give due notice and reasons for their absence. This form needs to be submitted 48 hours prior to your absence from a volunteering commitment.
Valid reasons for consideration in a leave of absence are:
- Health Reasons
- Family and Personal Emergency
- Cultural or Religious Ceremonies
- Academic Reasons
