Please fill out each page completely and fill out one form for each rider. If you have questions please call Sue Mrotek at 414-423-2800 ext. 0. Once you have completed the form, please drop off $60 (per rider) at any of the 5 Greendale schools and your child will be issued a bus pass. The pass will be sent to your child's school/homeroom teacher the week of May 22nd once we confirm there are enough riders to support the route. Each pass will have the student's name, stop and pick up time. Passes must be presented to the driver any time a student rides the bus. Students may only ride the bus they were assigned to and may not bring friends or guests on the bus.
