Secondary Math II - Course Disclosure
Please read through the course disclosure and answer the questions throughout the document.  This will show me that you have read and understand the course disclosure.  Also please have a parent/guardian complete the form on the last page.  If you have any questions, please don’t hesitate to ask me.

Tests
Each unit will end with a test.  Students will take the test in class and must finish before leaving that day.  If a student is absent for a test, the student should schedule with the teacher a time to take the test or it will be taken on the next day the student is present.  Retakes may be given according to the following guidelines:

1. Test retakes must be completed before the next unit test is administered.
2. The retake test will be a comparable version of the original test, not a duplicate.
3. If a student happens to earn a lower score on the retake test, he/she will retain the score from the original test.

A student will only be eligible for a test retake on the following conditions:

1. He/She shows effort on every problem of the original test.
2. He/She meets with Mrs. Clark for at least 10 minutes outside of class time to discuss the original test.

Assignments
On a regularly scheduled week, students will receive an assignment on four of the five days in class.  Student collaboration on assignments is encouraged but simply copying from another student’s assignment is not acceptable. Each assignment will consist of challenge questions and homework questions.  Frequently students will be given the opportunity to demonstrate their understanding by completing certain challenge and homework questions in class.  Students who can demonstrate their proficiency on these problems will be given the option of eliminating a certain number of problems from their homework assignment for that day.

Notes Binder
At the beginning of each unit, students will receive a bundle of skeletal notes to be completed as we move throughout the unit.  Each lesson has a corresponding page that will give students all of the information necessary to complete homework assignments and study for tests.  Students will receive points at the end of each unit for their notes binder, but all notes must be completed in their own handwriting to receive full points.  In the case of an absence, the student is responsible to obtain a copy of the teacher’s notes either online or in the classroom and fill in the pages that they missed.

Handing in Work
All assignments will be due the school day after they were assigned, unless otherwise specified by the teacher.  Work will be collected in class each day.  Students will receive points on each assignment for turning it in on time.  Assignments that are handed in after the work was due (for any reason) will be considered late.  Students who do not turn in assignments on time will not receive full points, unless they complete the additional homework problems that are provided for that purpose.  No late work will be accepted after the test has been administered for that unit.

Warm-ups
Every day students will start the class by participating in warm-ups.  These consist of simple math facts or puzzles that are important building blocks for learning math concepts.  If a student is absent, he/she will need to make up for the warm-up that was missed with Mrs. Clark outside of class or on a designated make-up day.  Students who are tardy will not be able to make up the warm-up for that day.
Absence
All absences, excused or unexcused, will be treated equally.  In all cases the absent student always has potential to receive full credit for the missed work but he/she may be required to complete the extra problems on the homework if the absence interferes with the student’s ability to submit completed assignments on time.  

Absent students will be expected to:

1. Complete the notes and homework with help from a copy of the teacher notes (available online or in class) or the notes of a classmate.
2. Come before or after school for additional instruction and assistance if necessary.

Tardy Policy
Students who are late to class will be given a tardy on his/her attendance record.  He/she may be required to make-up the work done in class that was missed.  After the student has received 5 tardies on their attendance record, they may be recommended for lunch or after-school detention.
Electronic Devices
The use of technology in mathematics is helpful, and in some cases necessary.  All of the electronic devices required for this course (calculator, clock, etc.) will be provided.  The use of any personal electronic device is prohibited, unless notified otherwise from the instructor.  Cell phones, tablets, laptops, iPods, ear buds, and any other personal electronic device will be confiscated on sight and returned at the end of class.  Devices that are confiscated more than twice will be delivered to the office.  Students who feel that it is absolutely necessary to check or respond to messages or phone calls may use a hall pass and leave the classroom to take care of their personal matters.  Students who are expecting an emergency call may ask for permission to be excused ahead of time.
