While you are completing this form, please consider 3 - 4 of the standards between question 1 and 20 which you focused on for this Professional Exchange, as we will ask you this at the end of this self-assessment.

We will not share this with anyone but you. We do have to report on progress for the Professional Exchange programme against the Professional Standards but we will collate and anonymise all data before sharing this with the Foundation.
