My name is Apollo Johnson. I have asked you to agree to be a volunteer in a study that I plan to conduct. Before I can accept your consent, I want to make known to you the following information pertaining to the project.

1. I am gathering information and studying the language differences between two generations of Chicago-area LGBTQ people. This has not been studied prior to my knowledge, I am the first one doing research on this topic. This research study will last until December 2018 and will only take a one-time session of 30-60 minutes for you. This will either be through an online questionnaire on Facebook private message or email, or by a face-to-face interview. I am asking you to consent on providing information regarding your personal identity, and your thoughts surrounding slurs in the LGBTQ community. This information will be only be kept on file until December 2020. When this information is reported in 2018 (via class presentation) and 2019 (via zine or small book), I will only use non-identifying information for you. I may use quotes from you during these, but nothing that would allow others to identify you.

2. A possible risk is that the topic may be triggering for you, as I will be asking questions about slurs. If you choose to do a face-to-face interview, there is a possibility you could be outed if someone overhears our conversation. This risk is nonexistent for interview via Facebook private message or email.

3. Other than the experience of participation in a research project, there are no particular benefits for you.

4. Every effort will be made to maintain the confidentiality of the records, except for any disclosure required by law. Confidentiality of records will be maintained by assigning pseudonyms to each participant. Only the researcher and faculty sponsor will have access to the participants’ real name and contact information.

5. If you find yourself stressed, depressed, and/or needing help after the participation period, talk to your doctor or medical provider about mental health services. I cannot provide compensation for any medical expenses.

If you wish to withdraw from this research project at any time, you may do so. There will be no penalty if you withdraw.
If any part of this research project changes, you will be notified immediately. Again, you may withdraw from the research project at any time.

5. You understand that if you am injured, no form of compensation is available. Medical treatment may be provided at your own expense or at the expense of your health care insurer, which may or may not provide coverage. If you have questions, you should contact your insurer.
