NOTE: THIS FORM AGREEMENT MUST BE READ AND SIGNED BEFORE THE PARTICIPANT IS PERMITTED TO TAKE PART IN ANY GB BASEBALL EVENT SESSIONS. BY SIGNING THIS AGREEMENT, THE PARTICIPANT AFFIRMS HAVING READ IT.

IN CONSIDERATION of being allowed to participate in GB Baseball events, activities and programs (including but not limited to try outs, practices, work outs, games and transportation related to such) under the auspices of British Baseball Federation and/or British Baseball Federation Limited (together “GB Baseball”), I, the undersigned, acknowledge, understand and agree that:

1. I RISK BODILY INJURY, INCLUDING PARALYSIS, DISMEMBERMENT, DISABILITY AND DEATH, AND while particular rules of the sport, skills, equipment and personal discipline may reduce this risk, THIS RISK OF SERIOUS INJURY DOES EXIST, AS WELL AS THE RISK OF DAMAGE TO OR LOSS OF PROPERTY; and

2. I KNOWINGLY AND FREELY ASSUME ANY AND ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES (defined below) OR OTHERS, and assume full responsibility for my participation. I recognise that an examination by a physician should be obtained by all participants prior to involvement in any exercise or physical program. If I have chosen not to obtain a physician’s permission prior to beginning this exercise or physical program with GB Baseball, I hereby agree that I am doing so at my own risk; and

3. I willingly agree to comply with the rules of the game and customary terms and conditions for participation. If however, I observe any unusually significant hazard during my presences or participation, or if I observe any concern in my readiness for participation, I will immediately bring such to the attention of the nearest employee, coach or official and remove myself from participation; and

4. I shall defend, indemnify and save harmless Releasees from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney fees, brought as a result of my participation (or that of my minor Participant, if applicable) and to reimburse them for any such expenses incurred. their directors, officers, officials, coaches, volunteers, employees, agents, and/or other participants, sponsors, principals, all of its agents, representatives, affiliates, suppliers, and, if applicable, the owners and lessors of the premises used for the activity (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, and/or LOSS OR DAMAGE TO PERSON OR PROPERTY, SUFFERED EITHER DIRECTLY OR INDIRECTLY UNLESS CAUSED BY THE NEGLIGENCE OR FRAUD OF GB BASEBALL (“Liability”), to the fullest extent permitted by UK law.
