If you intend to register children in both camps, submit a separate registration entry for each camp. Similarly, if you need to register more children than this form can accommodate, feel free to submit multiple registrations. You will get an email confirmation for each registration.

I hearby give my consent as the parent/guardian of the above named child/children to attend/participate in the Summer Camps and/or VBS programs at Beautiful Savior Lutheran Church at 11981 Pippin Rd. Cincinnati, Ohio 45231. My child and I hereby release, indemnify, and hold harmless the church, its employees and/or volunteers from any and all liability from any claim, injury, or loss sustained by or during my child's participation during Summer Camp and VBS Activities. I hereby authorize Beautiful Savior Lutheran Church to take and use photography and/or video of my child for crafts, keepsakes, or promotional purposes in any type of media and understand I will not be compensated for any such use.
